About Us
What is Anfield and Everton Primary Care Network?
Anfield and Everton Primary Care Network is a collaboration of 5 GP practices working together to support local people's health needs:
- Great Homer Street Medical Centre
- Islington House Medical Centre
- Mere Lane Group Practice
- Priory Medical Centre
- Townsend Medical Centre
It is also made up of a team of health professionals who work in and around the practices to increase the service we offer to patients:
- Dietician
- First contact physiotherapists
- Mental Health Practitioner
- Occupational therapists
- Care Coordinators
- Paramedics
- Pharmacists
- Social Prescribers
Our Values
In the summer, staff from the PCN and all five GP surgeries met to discuss a vision for the PCN - which values do we feel are most important, as we deliver care to patients?
Some of the main values were:
- Continuity (of care to the patient) - ensuring that patients receive high quality care however many people are involved in providing it.
- Inclusion (of all patients) - ensuring that we reach out to different patients in ways that are appropriate and useful them, in order to make sure they get the same quality of care as everyone else.
- Accessibility (of services) - ensuring that we offer as many different ways of making an appointment as possible, different times of appointments, and different types of care in and around the GP surgery.
