This website is for patients, to access information and support.

Your Primary Care Network (PCN) works with your local GP practices [see "Our Practices" page] to support your healthcare needs.

We provide a service of healthcare professionals [see "Our Patient Services" page] and work with other local organisations to provide information and education to support you [see "Future Patient events" page, "Recent Patient Events" page, and "Local Resources" page].

We invite feedback from you, about our service and the patient events that we organise or take part in.
